Home Services Laparoscopic Surgery for Ectopic Pregnancy
Ectopic Pregnancy occurs when a fertilized egg implants itself outside the uterus, most commonly in one of the fallopian tubes. This is known as a tubal pregnancy, but in rare cases, the egg can implant in other areas such as the ovary, abdominal cavity, or cervix.
In a normal pregnancy, the fertilized egg travels from the tube to the Uterus, where it attaches and begins to grow. However, in an ectopic pregnancy, Egg remains in fallopian tube, which means it cannot develop into a viable pregnancy. If left untreated, the growing tissue can cause the fallopian tube to rupture, leading to severe internal bleeding, which can be life-threatening.
Ruptured tubal pregnancy where patient presents with signs of internal bleeding.
When there is a risk of rupture of tubal pregnancy where the patient presents with severe pain.
When medical management is contraindicated.
When there is failed medical management.
When patient wants a definite management. Laparoscopic surgery provides a more immediate solution unlike medical management which requires multiple follow ups.
Also when there is recurrence of tubal ectopic pregnancy.
Laparoscopic surgery for a tubal(ectopic) pregnancy involves the removal of the ectopic pregnancy from the fallopian tube or, in some cases, removal of the affected tube itself.
Surgery is performed under general anaesthesia. A small incision is made near the umbilicus,
A trocar 10mm is inserted through the incision, and a laparoscope(a camera) is introduced to visualize the pelvic organs.
Two or three additional small incisions(5-10mm) are made in the lower abdomen to place accessory trocars for surgical instruments.
The specific approach depends on the extent of the damage, the patient’s
fertility wishes,
Salpingostomy (Preserving the Fallopian Tube)- A small incision is made
along the length of the fallopian tube(un ruptured tubal. pregnancy) over the
area containing the ectopic pregnancy.
Using delicate
instruments, the ectopic pregnancy is carefully removed from the tube, avoiding
damage to surrounding tissues. The incision site is typically left open to heal
on its own, reducing the risk of scar tissue formation.
Salpingectomy (Removal of the Fallopian Tube):- If the fallopian tube is
severely damaged (ruptured) or if preserving fertility is not a concern, the
affected fallopian tube may be removed.
The tube is
carefully dissected from the surrounding tissue and divided from the uterine
cornu (where the fallopian tube meets the uterus).
The tube is then
removed from the abdominal cavity, often through one of the laparoscopic ports.
After the ectopic pregnancy is removed and hemostasis is achieved the pelvic cavity is then irrigated with sterile saline solution, and any remaining blood or fluid is suctioned out.
The small incisions are closed with sutures or adhesive strips and covered with sterile dressings.
patient can go home 24 hr after surgeryIn cases of salpingostomy, serial blood tests for human chorionic gonadotropin (hCG) levels may be required to ensure complete removal of the ectopic pregnancy, as there is a small risk of persistent trophoblastic tissue.
