Lap abdominal encercalge involves the surgeon making 3 to 4 small incisions in the abdomen and then a video laparoscope, is used to guide the placement of a permanent stitch around the cervix. This stitch helps to hold the cervix closed during pregnancy, reducing the risk of miscarriage or premature delivery.

Indications

  • Women with repeated 2nd trimester miscarriages with no known cause.
  • Women with history of uterine abonormility
  • Women with a history of previous cervical surgery or trauma that may have weakened the cervix.
  • Women with an abnormal cervix, such as a cone biopsy or a trachelectomy.
  • Women with a short cervix, which is a risk factor for premature birth.

Procedure

It is performed under general anesthesia. The procedure involves the following steps:

  • Co2 is used to inflate the abdomen to ease the introduction of instruments into abdomen and avoiding injury to neighbouring organs.
  • A small incision in the belly button and a laparoscope, which is a thin, lighted tube with a camera attached to the end is put through which allows the surgeon to visualize the cervix and surrounding structures. 2 other small cuts are needed to facilitate instruments and to perform the surgery.
  • A synthetic tape is passed through the tunnel and around the cervix, just below the bladder . The suture is tightened to hold the cervix closed during pregnancy.
  • The instruments are then removed and the cuts are closed with sutures or surgical glue.
  • The patient is then monitored in the recovery room and may be discharged the same day or kept overnight for observation.
